Strive Tennessee receives referrals of those with disabilities from health care professionals. The referral source completes the referral form and provides a physician's script for recreational therapy.
Our client care manager will contact the patient and/or referral source to discuss additional concerns or issues not provided on the initial referral form in order to match the client with his/her best-fit Certified Therapeutic Recreation Specialist.
A Certified Therapeutic Recreation Specialist (CTRS), CTRS- eligible or TR assistant from Strive will set up an initial assessment meeting. Here, the client and therapist will develop goals and treatment plans. A schedule of future appointments will be made.
Strive Tennessee will provide invoices and request payment from the client's auto insurance, workers compensation insurance, medical insurance (very rare) or private payer.
5. FOLLOW UP
Strive Tennessee provides all necessary progress notes and paperwork to physician and/or treatment team or referral source (if needed) in order to maintain open lines of treatment communication. Doing such will ensure the client has successful sessions with therapist. Updates on physician prescriptions and current client treatment plans are actively monitored.
